• Yes! Vayu Health’s program is completely free for eligible members of Partnership HealthPlan of California or Health Net through the Enhanced Care Management (ECM) benefit in Humboldt, Shasta, and Stanislaus counties.

• ECM is a special Medi-Cal program that provides extra help for people who need more support managing their health.

• Vayu Health’s ECM Plus program offers personalized support to help you take control of your health and live your life to the fullest. Our unique care model includes social, wellbeing, and diabetes & nutrition Education.

•  If you qualify, all services from Vayu Health are covered at no cost to you—no fees, no bills, and no impact on your regular doctor visits.

If you’re already enrolled in an ECM program, you have the option to switch your ECM provider at any time by contacting your health plan provider. Participation in ECM is voluntary so you can always decide which ECM program is best for you. 

No, you don’t have to change your doctor. Our team works closely with your existing healthcare providers to support and reinforce your care plan—not replace it.

That’s up to you! We’ll contact you as often as you prefer—by phone, text, or in person. Our goal is to make it easy and comfortable for you to stay connected.

Yes. Our local Community Health Workers can meet with you in person—at your home or another place where you feel comfortable—to provide support and help connect you to local services.

No problem. Our team can work with you in your preferred language and provide translation support if needed. We want everyone to feel understood and supported.

• Your doctor focuses on your medical care. Vayu Health helps with everything that affects your health outside the doctor’s office—like managing stress, accessing food and medications, or learning how to take care of your diabetes day to day. We partner with your doctor to help you stay on track.

• You will be supported by a team that will provide wellbeing, social, and diabetes support.

Yes. Our team can help you understand your medications, make sure you’re taking them safely, and work with your doctor on insulin adjustments if needed. We can also help if you’re having trouble getting your prescriptions filled

There’s no set time limit. You can stay in the program as long as you’re benefiting from the support and continue to meet eligibility criteria.

We can help connect you to local programs and community resources for food, housing, and other essentials. Our goal is to support your overall wellbeing—not just your diabetes.
